UCLA Health Clinical Advisor LCSW - Ambulatory Care Management (Flexible-Hybrid) in Los Angeles, California

Description

Under the direction of the Assistant Director and Clinical Manager (LCSW) of Ambulatory Care Management, you will be responsible for working collaboratively with patients, Faculty Practice Group, community resources, other providers, managed care operations, the UCLA Health System and health plans to implement, coordinate, and monitor options and services to meet at-risk patients' healthcare needs through communication and coordination with providers, Comprehensive Care Coordinators, and staff at the practice site and in the appropriate UCLA Health System units to promote quality, cost-effective outcomes and appropriate level and mix of care. You will provide direct social work services, including assessment, intervention, and recommendations to individual patients and their families. Participate in interdisciplinary activities, comply with departmental standards regarding attendance, documentation, continuous quality improvement, statistics, departmental policies and procedures, and the Code of Ethics. Keep current with professional social work health care knowledge base and pursues further enhancement of job-related knowledge.

Salary Range: $105,298 - $125,844 Annual

Qualifications

A Master's Degree in Social Work from a school accredited by the Council on Social Work Education. Post-masters social work experience in a healthcare setting is highly desirable. Excellent oral and written communication skills. Strong relationship building skills. Excellent problem solving ability. Demonstrated skill in psychosocial assessments of patients/families, clinical interventions, and treatment planning with patients and families. Demonstrated work with interdisciplinary teams. Must have basic computer knowledge and word processing skills (use is frequent). Must operate telephone, fax, and copy machines (use is frequent). Must participate in at least one area of specialized practice annually. Must have strong problem solving skills. Ability to analyze problems and formulate plans, solutions and a course of action given changing circumstances. Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously. Strong ability to prioritize. Demonstrate proficiency in computer skills such as Windows computer function, computer navigation, and ability to use computerized patient care systems. Competence in navigating the internet/intranet and use of browser software such as Microsoft Explorer and applications such as Excel, Word, and PowerPoint. Excellent interpersonal, verbal and written communication skills. Ability to articulate clinical and non-clinical information to individuals of all levels of understanding. Ability to be flexible and work according to availability of office personnel, patient and resources required to coordinate care of patient. Clinical Advisor 3 - Must possess a current license (LCSW) with the California Board of Behavioral Sciences.
