Hillside Family of Agencies Care Coordinator II - Flex in Rochester, New York

We have an incredible opportunity for a Relief Mobile Care Coordinator II position at our Rochester, NY location. If you are looking for a great part-time opportunity to pick up some additional pay or an off-hours position (working a minimum of 13 hours between the hours of 3pm-11pm M-F and 7am-4pm on weekend days), this may be a perfect fit for you!

  • Relief/Flex position

  • 3:00pm – 11:00pm Monday through Friday

  • 7:00am-4:00pm Saturday and Sundays

  • In office position

    As part of the Mobile Crisis Stabilization Team, the Relief Care Coordinator will cover shifts in times of call offs/time off/staffing shortages within the program. The work entails triaging after hours’ crisis calls from Monroe County DHS and responds in person to foster homes as needed, to help deescalate the conflict and provide further immediate safety planning. The goal of the relief care coordinator will be to respond to immediate crisis, deescalate crisis/conflict, conduct assessment of risks, make immediate safety plans as needed, and establish short term permanency for youth in foster care by preventing same night home replacements and hospitalizations. The program follows a two-team approach as one care coordinator and one family peer advocate respond to crisis calls.

    Responsibilities :

  • Conduct crisis assessment of youth needs at time of crisis

  • Conduct immediate safety plans

  • Conduct site or home visits as needed to deescalate crisis/conflict in homes

  • Conduct crisis de-escalation as needed using verbal TCI techniques.

  • Conducts progress notes and program documentation within agency required timeframes to meet internal and COA audit standards

  • Participates in agency and team meetings and supervision with supervisor.

  • Relief staff will update the team via email of any client intakes/crisis responses before leaving shift at end of day.

    Requirements :

  • Bachelor's degree in social work, psychology, sociology, special education, human services or another human services related area of study

  • 1+ years of experience working in a youth/family centered mental health/psycho-social treatment environment

  • Must have thorough knowledge of the services provided, well developed advocacy skills

  • Must have a valid New York State Driver's License and meet agency driving and insurance standards

    $21.00 Minimum Rate per hour, $30.00 Maximum Rate per hour, based on experience.
