Direct Reports: Medical Case Manager ( s ) , Peer Navigator , Matrix Supervision – Registered Nurse, HIV Services

Purpose : Assist in the development, implementation, management, and evaluation of the Age Related Changes with HIV (ARCH)/ People Aging with HIV (PAWH)/ grant . Provide supervision and leadership to PAWH/ARCH team members in case management .

  • Program Development and Evaluation

  • Assist Manager, Grant Funded Services in the administrative, planning, development, and implementation functions of the Care Management d epartment .

  • Develop and maintain relationships with community agencies and medical facilities to obtain new referrals and ensure unduplicated services for existing patients/clients .

  • Work with grant and department leadership to design and monitor performance and quality standards, including outcome evaluations, productivity and workload targets, peer review and client/patient record reviews .

  • Ensure that staff members meet the contract requirements, deadlines and expectations of the agency, our department and our funders .

  • Review patient /client complaints, as directed by Manager, Grant Funded Services, to identify possible/probable root cause, and take steps necessary to prevent recurrences in the future .

  • Supervision and Performance Management

  • Participate in the selection , hiring and orientation of assigned direct reports .

  • Ensure staff development, assess competencies, and provide guidance as needed .

  • Clearly articulate expectations and manage to those expectations through positive feedback, coaching and progressive discipline if required .

  • Management of staff time clocks, work schedules, etc.

  • Review of documentation completed on behalf of and about patients/clients, including assessments, service plans and log notes.

  • A ddress performance issues through coaching/ counseling, performance improvement plans, and progressive discipline as r equired .

  • Demonstrate leadership, good judgment and sound decision making when handling situations that require supervisory intervention .

  • Complete yearly performance evaluations for assigned direct reports .

  • Provide matrix supervision to Registered Nurse, HIV Services, working with the Director of Nursing to ensure regular joint supervision meetings and communication regarding performance of job responsibilities specific to ARCH/PAWH responsibilities .

  • Case Management and Outreach Services

  • Model and facilitate effective teamwork and collaborative behaviors towards Trillium coworkers and interagency colleagues, which includes coverage of staff absences and providing good customer service .

  • Perform outreach and develop partnerships in the community to expand ARCH/PAWH services in the communities we serve.

  • Provide patient centered, quality drive n , culturally appropriate case management to assigned patients /clients in compliance with agency policies and funder requirements .

  • Build relationships with patient s/clients and all members of the treatment team to support continuity of care and promote health.

  • Document each patient /client interaction comprehensively and concisely in accordance with funder standards and organizational expectations.

  • Advocate for patient/client access to entitlement programs, treatment facilities, and other needed community services to address medical and behavioral health and social determinant of health needs identified by them.

  • Promote patient access into internal agency services including but not limited to support groups, housing services, nutrition & health education, behavioral wellness, substance use treatment and medical care.

  • Adhere to all government and funder regulations

  • Assist in identifying additional resources that help support the needs of pa tients/clients .

  • Assist in the completi on of necessary program reports/audits.

  • In collaboration with the Manager , Grant Funded Services, support all communications with grant contract managers, via reporting, calls, and meetings on progress as applicable.

  • Monitor grant deliverables and budget, implement strategies to successfully execute as applicable.

  • Participates in the de velopment of Ca r e Management department and grant specific policies and procedures .

    Qualifications

  • Bachelor's degree in social work, human services, or closely related field.

  • Experience working in a management/supervisory role within a social service, nonprofit, grant management, or healthcare setting preferr ed .

  • Familiarity with regional HIV primary care, mental health, substance abuse and other services and resources.

  • Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations.

  • Effective communication and documentation skills.

  • Cultural and linguistic competence for the target population.

  • Must have a positive attitude, demonstrate initiative and professionalism. 

  • Must be accustomed to diversity.

  • The ability to maintain confidentiality is essential.

  • Experience in managing client case load assignments and functioning within a management team.

  • Must be able to work flexible hours including weekdays, evenings and weekends when needed. Must have a reliable vehicle, a valid NYS driver’s license, and proof of required automobile insurance.

    Physical Requirements

    While performing this job, the employee must stand, sit, and walk.   Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
