South Middlesex Opportunity Council Triage Case Manager Supervisor in Worcester, Massachusetts

SUMMARY

Oversee all operations and staff dedicated to providing initial screening and triage case management services to clients residing in emergency shelter in need of stable and permanent housing placement. These responsibilities include the supervision of triage case managers, the diversion case manager, the coordination of all client engagement services, and the re-housing plan development and implementation.

PRIMARY RESPONSIBILITIES

  • Ensure effective initial triage is conducted, required documentation is collected, and diversions are tracked.

  • Ensure appropriate coordination of services with community providers and systems by developing and maintaining effective working relationships with all appropriate service providers.

  • Review and audit all operational and staff documentation so as to ensure compliance with contracts, company policy & procedure, and other governing documents.

  • Supervise program activities to ensure they provide opportunities and motivate clients to enhance their quality of life and ability to achieve self-sufficiency.

  • Partner with the Housing Stabilization Supervisor and Triage Services Program Manager to ensure and provide effective communication between Triage and Housing Stabilization.

  • Participate in daily team meetings to assure that re-housing plans are monitored and effectively carried out.

  • Assist your team with identifying and accessing all resources for successful housng placement both within the MSOC Housing portfolio and with extenral landlords and programs. Maintain a consistent focus to minimize the length of shelter stays.

  • Participate in the development of program proposals, timelines, and implementation strategies as well as the program budget.

  • Provide emergency and on-call response coverage to program staff in coordination with Triage Services Program Manager for urgent triage needs.

  • Present to community groups and other service providers as required

  • Participate in, and may lead various client groups or community committees

  • Provide transportation to clients as necessary

  • Manage the operational and fiscal activities of the department including staffing levels, budgets, and financial goals.

  • Recruit, manage, develop, and evaluate staff by providing timely mentoring, training, and performance management. Complete the required paperwork in accordance with company policy.

  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.

  • Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.

  • Maintain confidentiality of client, employee, and agency information in accordance with federal and state laws and funder requirements.

  • Ensure compliance with program/department, agency, and/or funder requirements, as well as, SMOC policies & procedures.

  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • B.A. in Human Services, Masters Degree preferred, CADAC Training with internship preferred.

  • Minimum 5 years of leadership experience in providing services to clients dealing with substance abuse and homelessness. More than 7 years of directly related experience may substitute for B.A.

  • Strong supervisory, planning, and organizational skills

  • Excellent verbal and written communication skills

  • Must be able to work independently as well as on a team.

  • Ability to manage complicated relationships with external service providers and funding sources.

  • Strong Presentation skills.

  • Valid driver’s license and ability to meet insurance standards required.

ORGANIZATIONAL RELATIONSHIP

  • Directly reports to the Assistant Director of Greater Worcester Housing Connection (GWHC).

  • Direct reports of this position are Diversion Specialist Triage Case Managers and Assessment Case Managers.

PHYSICAL REQUIREMENTS

  • Ability to drive.

WORKING CONDITIONS

Some local travel may be required. On-call nights and weekends may be necessary. As part of the responsibilities of this position, the Triage Case Manager Supervisor will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services.
